2025-2026 Enrollment Forms
We are converting to a partial online registration process for the upcoming school year. If you are are interesting in enrolling your student to Green Lake School, please fill out the enrollment form below and return it to the school. You can email sondallek@glsd.k12.wi.us, fax 920-294-6589, or drop it off in the school office.
