High School and Middle School Athletic Programs

This year, we are using Bound for all athletic registrations. You will need to create a family account on Bound before registering your child for any sport. Once your account is set up, you can register your child for all sports throughout the school year. Please note that all forms will be completed through Bound and will not be available in the office. If your child had a physical last year, you must upload a copy to your Bound family account for each child. If you need assistance or a copy of the physical form, please contact Amy Dornfeld at dornfelda@glsd.k12.wi.us and she will forward you a copy.

Please note that transportation forms and fees must still be completed through the Green Lake office. All registration, paperwork, and fees must be finalized before your child can begin practice.

Student Alternate Transportation Liability Waiver
(if a student would like to drive themselves to practice in Princeton.  NOTE:  Student are NOT allowed to drive any other students unless they are a member of their household)

Parent Driver Transportation Liability Waiver
(This form must be filled out if you would like to drive your student home from games/meets.)

Sport participation fees:

High School Fees - $30 per sport

Middle School Fees - $15 per sport
